Nominate a deserving child or mom-to-be for A Cake To Remember. Rules Apply.

1. One nomination per child up to age 18. Moms-to-be are also included.

2. Parent/Guardian, teacher, counselor, or case manager can nominate.

3. Nominations run from the 1st of every month until the 15th at 11:59 pm for the following month. For example, if your nominee has a birthday on June 6, you must submit your nomination form by May 15 at 11:59 pm.

4. No late submissions will be accepted.

5. At this time, nominees should be within a 25-mile radius of Gurnee, IL.

3. In your own words write or upload a video detailing why your nominee deserves A Cake To Remember.

1. Winner will be selected on or about the 20th of each month.

2. Winner will be selected based on the completion of entry rules.

A panel of judges will review all submissions and select the month's winner. In the event of a tie, both submissions win.

3. Winner will be notified via the email or phone number provided at time of entry.

4.  Winner will have 48 hours to accept the prize or forfeit.  In the event the winner cannot be reached,a runner-up will be selected and awarded Grand Prize.

 Upon acceptance of prize, winner agrees to have image or video placed on company website for future marketing purposes.
